How Do Chiropractors Approach Sciatica?
If you’re experiencing pain, numbness, or tingling that radiates from your lower back down your leg, you’re not alone. Sciatica is one of the most common forms of back pain, affecting approximately 40% of the American adult population at least once in their lives, and it often recurs. Understanding that sciatica refers to an issue with a particular nerve, which can be impinged for various reasons, is critical. At Spine Spot Chiropractic, Dr. James Fraser is dedicated to diagnosing the root cause of your sciatica and developing a comprehensive treatment plan to support your body’s natural recovery with minimal pain and intervention.
Understanding the Sciatic Nerve
What Is the Sciatic Nerve?
The sciatic nerve is the largest nerve in the body, with one nerve running down each leg. These nerves consist of multiple nerve roots that emerge from the spinal cord in the lower back and traverse the pelvic muscles before typically splitting behind the knee to continue into the foot. Compression of the sciatic nerve can lead to pain or loss of function along its length, often near its roots or as it weaves through the pelvis.
How Can the Sciatic Nerve Be Injured?
The sciatic nerve can be compressed due to swelling of surrounding tissues following an acute injury, though it is often the result of progressive degenerative conditions. Typically, only one sciatic nerve is compressed at a time, consisting of five roots—two from the lumbar spine and three from the sacrum. Compression may occur if a disc between the lumbar vertebrae ruptures, causing its gelatinous core to bulge and press on a nerve root.
Furthermore, misalignment from joint fractures connecting the lumbar vertebrae can compress the nerve or trigger inflammation in nearby muscles. Compression can also happen lower down if any muscles or tendons in the pelvic region become strained. The lumbar vertebrae connect to the upper femur through a wedge-shaped muscle known as the piriformis. While the sciatic nerve generally passes around this muscle, some people's nerves may pass through or wrap around it. Swelling in the piriformis can pinch the sciatic nerve, with additional inflammation in surrounding muscles worsening the condition.

Comprehensive Assessment and Treatment Plan
Dr. Fraser employs thorough manual, range-of-motion, and imaging tests to confirm whether sciatic nerve impingement is the primary cause of your discomfort. The focus then shifts to relieving pressure on the nerve through various methods, including:
- chiropractic adjustments: Applied to both joints and soft tissues, adjustments aim to restore proper alignment and reduce pain.
- Anti-Inflammatory Therapies: To alleviate inflammation and promote healing.
- Therapies: Electric muscle stimulation, low-level laser therapy, and ultrasound to enhance blood flow and provide vital nutrients to injured muscles.
Long-Term Relief and Prevention Strategies
While sciatica typically resolves within six weeks, preventing recurrence requires reducing stress on your spinal discs, joints, and hip muscles. This may involve:
- Strengthening Core Muscles: To handle more weight effectively.
- Weight Management: To alleviate unnecessary stress on the back.
- Stretching Exercises: To enhance flexibility in the hip muscles.
- Posture Guidance: To distribute weight evenly while standing and sitting.
