Time of Service Policy
Our TIME OF SERVICE FEE shall be paid by cash or card at the time of treatment. This price reflects our savings on bookkeeping and billing service fees.

Adjunctive Physiotherapies & Modalities
To accelerate tissue healing and support your structural adjustments, Dr. Fraser may recommend specific targeted therapies.
Physiotherapies: $30. (Please Note: Physiotherapy modalities are adjunctive add-on services and are billed in addition to your standard or extended office visit fee).
Emergency & After-Hours Care
- After-Hours Consultation: $200 base fee, plus the standard cost of any specific clinical services or treatments rendered during the visit.
Pre-Paid Care Plans
For patients committed to long-term structural correction or maintenance, Dr. Fraser offers pre-payment plans for maximum clinical value. Compliance Note: Because these plans reflect a heavily discounted cash rate, they are strictly ineligible for SuperBill generation or insurance reimbursement.
Refresh Care Plan (4 Visits): $295.
This reduces the cost to $73 per visit (Saving $45). One Year Expiration Date.
Locals Special Care Plan (12 Visits): $780.
This reduces the cost to $65 Per visit (Saving $240). Two Year Expiration Date.
Altitude Armor Care Plan (24 Visits): $1,320.
This reduces the cost to $55 per visit (Saving $720). NO Expiration Date.

Insurance, Medicare, & SuperBill Policies
Our clinic operates as an out-of-network, direct-pay provider, allowing Dr. Fraser to dictate the highest standard of care without third-party interference. Patients may utilize their out-of-network benefits subject to the following strict compliance protocols:
- SuperBill Eligibility & Out-Of-Pocket Requirements: SuperBills are only generated for patients who pay the full standard Time of Service rates. We cannot and will not issue a SuperBill for visits rendered under a discounted Pre-Paid Care Plan.
Commercial Insurance: Upon Request, we can provide you with a SuperBill to submit to insurance for reimbursement directly.
Medicare: We can also provide a SuperBill for Medicare patients to seek reimbursement directly.
Good Faith Estimate
Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
